Gangnam-gu joins hands with six medical institutions on an ‘integrated care support program for discharged patients’

To narrow the care gap that can follow discharge, Gangnam-gu has joined with local medical institutions to build an integrated support system.
On the 25th, Gangnam-gu in Seoul signed a memorandum of understanding (MOU) at the district office with six local healthcare institutions to carry out the ‘Integrated Care Discharged Patient Support Program’. Samsung Medical Center, Gangnam Severance Hospital of Yonsei University College of Medicine, St. Peter's Hospital, Nanoori Hospital, Medrex Hospital and Gangnam-gu Municipal Haengbok Long-Term Care Hospital took part.
Under the agreement, each healthcare institution will identify and assess at an early stage patients who have been discharged or are due for discharge and who need integrated care, and then refer them to Gangnam-gu. The district will comprehensively review the health status and care needs of those referred and, through an integrated support meeting, connect and provide tailored services in stages.
In particular, Gangnam-gu will integrate local health and welfare resources — home-visit medical care and nursing, long-term care services, daily living support and housing referrals — into a single system of support. Through this it plans to build a preemptive care system that minimizes gaps in care immediately after discharge and prevents unnecessary readmission.
Gangnam-gu Mayor Cho Sung-myung said, “Discharge is the end of treatment, but care must continue afterwards,” adding, “Through this agreement we will provide continuing support so that residents can carry on their daily lives with peace of mind in the place where they live.”
