What to prepare

  • Your exact name as shown in your passport (in English) – bringing your passport with you makes this easier.

  • Vaccination record booklet (baby book)

  • One copy of the form requested by the overseas school or country you are going to

  • The documents and health screening items required may differ depending on the country or institution you plan to visit.

  • Please check before booking your screening so that your customized health screening covers everything required.

We also carry out medical aptitude tests (aptitude tests for elderly drivers), health certificates for licence issuance, firearms examinations, health screenings for healthcare workers, and on-site screenings and screenings for international students (including working holiday applicants) for those who need screening documents for submission to immigration offices and other institutions.
