Gangnam supports discharged patients in need of integrated care

Gangnam-gu in Seoul announced on the 26th that on the 25th it had signed a memorandum of understanding (MOU) at the district office with six local healthcare institutions to carry out the ‘Integrated Care Discharged Patient Support Program’. The signatories are Samsung Medical Center, Gangnam Severance Hospital of Yonsei University College of Medicine, St. Peter's Hospital, Nanoori Hospital, Medrex Hospital and Gangnam-gu Municipal Haengbok Long-Term Care Hospital.
Under the agreement, the healthcare institutions will identify and assess at an early stage those patients who have been discharged or are due for discharge and who need integrated care, and refer them to Gangnam-gu. The district will comprehensively review each person's health status and care needs and, through an integrated support meeting, connect and provide the necessary services in stages.
In particular, the district plans to integrate local health and welfare resources — home-visit medical care and nursing, long-term care, daily living support and housing referrals — into a single system, thereby realizing a preemptive care system that minimizes gaps in care immediately after discharge and prevents unnecessary readmission.
Gangnam-gu Mayor Cho Sung-myung said, “Discharge is the end of treatment, but care must continue afterwards,” adding, “Through this agreement we will provide continuing support so that residents can carry on their daily lives with peace of mind in the place where they live.”
