Gangnam-gu supports discharged patients... integrated care agreement signed

On the 25th, Gangnam-gu in Seoul signed a memorandum of understanding (MOU) with six healthcare institutions in the district at the district office, in order to reduce gaps in care after discharge and strengthen community-based care. The participating institutions are Samsung Medical Center, Gangnam Severance Hospital of Yonsei University College of Medicine, St. Peter's Hospital, Nanoori Hospital, Medrex Hospital and Gangnam-gu Municipal Haengbok Long-Term Care Hospital.
Under the agreement, the healthcare institutions will identify at an early stage those patients due for discharge who need integrated care and refer them to Gangnam-gu. Gangnam-gu will comprehensively review each patient's health status and care needs, draw up a tailored support plan, and then connect and provide the necessary services in stages.
Gangnam-gu plans to integrate local health and welfare resources into a single system and, through home-visit medical care and home-visit nursing, long-term care, daily living support and housing referrals, to build a preemptive care system that minimizes gaps in care immediately after discharge and prevents readmission.
The agreement is seen as putting into practice on the ground the policy direction of the ‘Act on Integrated Support for Community Care including Medical and Long-term Care’, which takes effect this March, and as an important step in linking medical care with social care so that patients can go on living stably in the community after hospital treatment.
Gangnam-gu Mayor Cho Sung-myung said, “Discharge is the end of treatment, but care must continue afterwards,” adding, “Through this agreement, hospitals and the local government will work together to provide seamless care after discharge and support residents so that they can carry on their daily lives with peace of mind.”
